How to Register
Patients MUST complete both the GMS1 Form and New Patient Questionnaire Form. Copies of the relevant forms can be found below for patients to print at home and complete. You may also come to one of our practices and request the forms in person.
Please also bring proof of address and photo identification when registering. On completion and return of all forms we will register you as a new patient.
Please note the practice cannot accept your registration request without all of the appropriate documents.
