Join our Patient Participation Group

We would like to know how we can improve our service to you and how you perceive our surgery and staff.

To help us with this, we have set up a patient participation group so that you can have your say.

This is a group of patients with different backgrounds who meet with the practice staff up to 6 times a year. The practice value this input from patients on how to develop our services.

If you would like to join the forum, or just be a member who receives the minutes of the meetings and contacts us occasionally, please complete the online form below or alternatively you can request a form from Reception.
